目的  探讨急性肠系膜缺血疾病后血清酶谱变化特点,期待通过血清酶谱变化检测,有助于急性肠系膜缺血疾病的早期诊断。

方法  分别建立兔急性肠系膜动脉缺血、急性肠系膜静脉缺血、急性机械性肠梗阻、急性绞窄性肠梗阻、急性胃穿孔、急性胰腺炎、假手术组、空白对照组的动物模型,分别在01369h抽取静脉血化验血清酶谱,统计对照分析酶谱变化,找出不同模型间的酶谱变化曲线差异。

结果  急性肠系膜缺血疾病组的血清酶谱明显升高。ALT(谷丙转氨酶)在鉴别急性肠系膜缺血和空白对照、假手术组、胃穿孔、机械性肠梗阻、绞窄性肠梗阻之间有统计学意义;ALP(碱性磷酸酶)在鉴别急性肠系膜缺血和绞窄性肠梗阻、急性胰腺炎之间有统计学意义;CK(磷酸肌酸激酶)、HBDH(羟丁酸脱氢酶)、LDH(乳酸脱氢酶)在鉴别急性肠系膜动脉缺血和绞窄性肠梗阻之间有统计学意义;CK在鉴别急性肠系膜动脉缺血和机械性性肠梗阻之间有统计学意义;r-GT(谷氨酰转移酶)在鉴别急性肠系膜动脉缺血和急性肠系膜静脉缺血之间有统计学意义;ALPAMY(淀粉酶)、r-GT在鉴别急性肠系膜静脉缺血和急性胰腺炎之间有统计学意义;CKAMYr-GT在鉴别急性肠系膜静脉缺血和胃穿孔之间有统计学意义。AST(谷草转氨酶)在鉴别急性肠系膜缺血与其他疾病组间没有统计学意义。

结论  血清酶谱变化有助于急性肠系膜缺血性疾病的诊断。

Objective: To study changes of serum enzymes of acute mesenteric ischemia disease, looking forward to contribute to the early diagnosis of acute mesenteric ischemia by serum enzymes change detection.

Methods: Establish the rabbit acute mesenteric artery ischemia, acute mesenteric venous ischemia, acute mechanical ileus, acute strangulated intestinal obstruction, acute gastric perforation, acute pancreatitis, sham group, blank control group animal models, in the 0,1,3,6,9 h testing serum enzyme, statistical control analysis enzyme change identify zymography curve differences between different models.

Result: Acute mesenteric ischemia disease group was significantly higher serum enzyme. Elevated ALT (alanine aminotransferase) statistically significant identify acute mesenteric ischemia and blank control, sham group, gastric perforation, mechanical ileus, strangulated intestinal obstruction; Elevated ALP (alkaline phosphatase) statistically significant identify acute mesenteric ischemia and strangulation intestinal obstruction, acute pancreatitis . Elevated CK(creatine kinase),HBDH(hydroxybutyrate dehydrogenase),LDH(lactate dehydrogenase) statistically significant identify acute mesenteric artery ischemia and strangulated intestinal obstruction; Elevated CK was statistically significant identification between the acute mesenteric artery ischemia and mechanical ileus; Elevated r-GT (glutamyl transferthe enzyme) statistically significant identify acute mesenteric artery ischemia and acute mesenteric venous ischemia; Elevated ALP, AMY (amylase), r-GT statistically significant identify acute mesenteric venous ischemia and acute pancreatitis; Elevated CK, AMY, r-GT statistically significant identify acute mesenteric venous ischemia and gastric perforation. Elevated AST (aspartate aminotransferase) was not statistically significant identification of acute mesenteric ischemia and other diseases .

Conclusion: Serum enzymes changes are helpful to diagnose acute mesenteric ischemic diseases, but still needs further research to demonstrate and prove.
